Enteric protozoa continue to be the most commonly encountered parasitic disease; they affect millions of people each year and cause significant morbidity and mortality worldwide. Among them, Cryptosporidium, Giardia lamblia and Entamoeba histolytica are the main etiological agents.
Cryptosporidium is a protozoan widely recognized as the cause of waterborne and foodborne diarrheal disease outbreaks globally. Infections result from ingestion of fecal-contaminated food or water, direct person-to-person contact, or zoonotic spread.
The main symptom is watery diarrhea, often accompanied by other signs of gastrointestinal distress such as abdominal cramps and, less commonly, nausea, anorexia, fever, and malaise.
Giardia lamblia is a flagellated protozoan that causes gastroenteritis of the small intestine. Giardia enteritis is spread worldwide and is usually contracted through contact with contaminated water but transmission can also occur through the ingestion of contaminated food or direct person-to-person contact.
Complications include hypokalemia, malnutrition, stunting, cognitive impairment, arthritis/myopathy, irritable bowel syndrome, and chronic fatigue.
Entamoeba histolytica is a protozoan that causes intestinal amoebiasis and extraintestinal manifestations. Amebiasis is acquired by direct person-to-person contact (faecal-oral transmission) or indirectly through food or water. The symptoms range from mild diarrhoea, intermittent constipation, flatulence, crampy abdominal pain to severe dysentery; extraintestinal infections include liver abscesses.
Molecular assays based on Real-Time PCR technology are indicated for the identification of these pathogens, thanks to their rapid response times, high sensitivity and specificity and are now a routine tool in diagnostic laboratories.
