The translocation t(15;17)(q22;q21) is frequently associated (more than 90% of cases) with Acute Promyelocytic Leukemia (APL), a subtype of Acute Myeloid Leukemia (AML) with M3 cytomorphology. APLs account for 10-15% of AML cases. The translocation t(15;17)(q22;q21) derives from the fusion of the PML gene (promyelocytic leukemia gene), located on chromosome 15q22 and the RARa gene (retinoic acid receptor α) located on chromosome 17q21. The break point in the RARa gene is located within a region of 15 Kb of intron 2; the break point in the PML gene, on the other hand, can occur in three possible regions: intron 6 (55% of cases), intron 3 (40% of cases) and exon 6 (5% of cases). At each break point, a particular PML-RARa isoform is associated, called bcr1, bcr3 and bcr2 respectively. The resulting fusion protein is a transcriptional repressor that hinders the differentiation of hematopoietic stem cells, which remain blocked at the promyelocyte stage. Treatment with ATRA (All Trans Retinoic Acid) is very effective in APL therapy by acting as a promoter of the differentiation process.
